The Infectious Diseases Center (IDC) is the result of a collaborative effort of The University of Cincinnati Medical Center and the Greater Cincinnati Health Alliance to evaluate and treat persons with HIV infection (Human Inmmunodeficiency Virus or AIDS virus), and persons with any other infectious diseases. Examples of the latter include infections in transplant patients, bone infections (osteomyelitis), recurrent sino/pulmonary infections, rheumatic fever, meningitis, various fungal infections (e.g. histoplasmosis, aspergillosis, cryptococcosis), tuberculosis and infections related to foreign travel. What to expect at your appointment: New patient scheduling may vary according to your medical need and where you will receive your primary care. In some instances, your first visit will be with an IDC physician. In other cases, your first appointment will be scheduled with a new patient specialist who will describe patient care and research programs and may obtain blood specimens to further evaluate your medical problem. A second appointment will be scheduled with the physician. During the physician visit, you will have a complete medical history and physical examination. Additional tests may be ordered. The results of your tests will be fully explained, and you and the physician will develop your follow-up care plan. If you have a primary care physician, a consultation letter will ONLY be sent with your permission. At the IDC, other health care providers are available to serve you, such as nurse clinicians, health educators, pharmacists, social workers, financial counselors, and mental health providers. Location and parking: The IDC is located at The University Hospital on the first floor of Holmes. Convenient outpatient parking is provided in an adjacent parking lot. Bring your parking ticked to the IDC; we can validate it for a reduced parking fee. Enter the building on the ground floor, and take the elevator to the first floor, then follow signs to the IDC. If you have problems arranging transportation, please call us at (513) 584-6977, and we will try to assist you. Personal Information: When we schedule your initial appointment, we ask you questions about your address, age, social security number, and health insurance or HMO* authorization number. To verify this information, you will be asked to fill out a Patient Information Form. If you have health insurance, please bring your insurance identification card so that we may make a copy for our records. *If you belong to an HMO, you should receive an authorization number from your primary care physician with an explanation of the number of visits and the extent of coverage they will provide for this treatment. Patient Confidentiality: Personal records from your treatment at the IDC are confidentially maintained at our facility. Your medical records at the IDC can be released only with your written permission. We abide by the rules set forth by the Health Insurance Portability and Policy Act (HIPPA). |
